What are Warts?

A wart is an intra epidermal or skin tumor caused by a variety of viruses. The many varieties of warts that exist are named by their appearance or location. A wart is a small, circumscribed tumor of the outer or horny layer of the skin. They are flat or elevated from the surrounding skin and are firm.

A wart is a virus classified within the Human Papilloma Viruses (HPVs) family, which represents a group of more than 100 types of viruses. They are called papilloma viruses because certain types cause warts, or papillomas, are benign (non cancerous) tumors.

Warts vary in size and shape and may be accompanied by pain, especially if they occur on the feet, which are known as plantar warts.

The Human Papilloma Virus belongs to the family of viruses that causes warts and has been implicated as a possible cause of genital cancers.

Common warts are caused by HPV types 2, 4, and 7. The warts are round firm skin- colored papules with a hyperkeratotic papillary surface. Sizes range between 1 to 10 mm. The hands and fingers are common sites. Thrombosed capillaries which appear as black dots can be seen on the surface of some warts. You can see the dark spot on the right of the wart in this picture. That is a capillary filled with blood feeding the skin and virus. This is why it is so painful to remove warts from any skin area. The virus is actually in the skin that nerves are attached to, that is fully alive.

HPV, commonly known as the "wart virus", is a microscopic virus particle that infects the skin. HPV is one of the most common infections in the world, infecting approximately 40% of all humans.

In the case of HPV, the infection is actually localized directly to the infected point in the skin, as opposed to the herpes simplex virus. Unlike HPV, the herpes virus goes through the skin and into the nerve cells, traveling up the nerve cell connection to the nerve ganglia by the spinal cord, where the virus lives. With HPV, the infection is actually in the skin.

HPV is contagious and transmitted through contact by an infected piece of skin with a non-infected piece of skin. The presumption is that the non-infected area has to have a small, perhaps microscopic, break that allows a tiny amount of tissue from the infected HPV area to get into the non-infected area, allowing an infection to set up.

Therefore, if a person with a wart anywhere on that person’s body were to place that area in contact with the genitalia of the other person, this could cause the wart virus to be transmitted to the genital area of the partner. This applies to warts on fingers as well as warts on the genitals of the infected partner.

Different types of HPVs cause different types of warts that may grow on various parts of the body such as hands and feet or around the mouth and genital area. Warts appear as single bumps or in clusters, some having a cauliflower structure.

Plantar Warts

Plantar warts are confined to bottoms or sides of the feet. They are generally flat due to being pushed upward into the foot by the body's weight. They are frequently very painful. This is because the wart virus is actually part of your skin and the nerves are intertwined with the skin tissue. The wart is actually much larger than what is visible to the naked eye. You can push on it and see more of a yellowish color to find out how big it actually is. Because it is on the bottom of your foot, it makes it very difficult and painful to get rid of.

Blood vessels actually form in the virus tissue to keep it alive. You might see a black spot in the center of it. That is the blood vessels feeding the virus. Plantar warts are very contagious due to the cells being sloughed off at a rate higher than any other wart and the virus remains stable in a favorable environment such as a warm, wet gym shower floor. Many athletes contract plantar warts in a similar fashion as they contract athletes foot (a fungal disorder). Plantar warts are often confused with bunions. Diabetic patients have a high incidence of plantar warts and athletes have the highest incidence.

Plantar warts result from HPV type 1 infection of the plantar aspect of the foot. They are slightly elevated skin-colored papules or nodules with a surface devoid of skin lines. You can see the dark spot on the right of the wart in this picture. That is a capillary filled with blood feeding the skin and virus.

Flat warts are smaller and smoother than other warts. They tend to grow in great numbers - 20 to 100 at any one time. They can occur anywhere, but in children they are most common on the face. In adults they are often found in the beard area in men and on the legs in women. Cuts from shaving spread the virus quickly.

Flat warts are caused by HPV types 3 and 10. They are round or oval shaped, slightly elevated and sharply defined. Flat warts appear as skin colored sometimes pink or brown papules with a flat surface. Size can vary from 2 to 4 mm. Multiple lesions generally occur. Auto inoculation (spreading the virus) by scratching can result in linearly grouped lesions. The face and hands are common sites.

Traditional Treatments for Common, Flat and Plantar Warts

• Laser Surgery--Warts can be removed through laser but this treatment may leave a scar, and can be very painful. A stream of laser light is directed at the infected site to destroy the wart. It may not destroy viral root and the wart may return.
• Electro-cauterization--A heated electric wire is applied to the growth and destroys the wart. One can imagine how much this treatment hurts.
• Canthardin Treatments--Your doctor may use cantharidin on your warts. With this treatment, the doctor paints the chemical onto the wart. Most people don't feel any pain immediately when the chemical is applied to the wart. You'll experience severe pain and blistering of the wart in about 3 to 8 hours. After treatment with cantharidin, a bandage or tape is put over the wart. The bandage can be removed after 24 hours. When mixtures of cantharidin and other chemicals are used, the bandage is removed after 2 hours. When you see your doctor again, he or she will remove the dead skin of the wart. If the wart isn't gone after one treatment, your doctor may give you another treatment. This method is extremely painful and uncomfortable with a varying success rate.
• Surgical Excision--First the wart area is injected with Lidocaine to numb it. This procedure alone is very painful. Then the wart is removed by cutting out the infected area, usually leaving a scar.
• Cryosurgery or Freezing with Nitrogen--Warts may be frozen with liquid nitrogen but will require several visits to the doctor's office. Liquid nitrogen is applied directly to the wart in order to freeze the growth and destroy the infected tissue. This procedure is very painful and in some cases requires an injection of Lidocaine to numb the area before the freezing procedure can begin. The numbing treatment sometime hurts as much as the freezing, especially with plantar warts. There is generally scarring with this procedure.
• 40% Salicylic Acid, Nitric Acid, etc.--Warts can be treated with salicylic or nitric acid solution but you must apply the acid every day for many weeks. Extreme burning, scarring and enlargement of wart tissue are common. This is a common method used, and there are many over the counter products available that contain these acids. Unfortunately, they are only about 30% effective in wart treatment.
